Heart Failure - Introduction

Lanarkshire has a Heart Failure Nurse Specialist at each of its three general hospitals – Hairmyres, Wishaw and Monklands.

The heart failure nurse provides support to patients in their own homes, in clinics and in the hospital and will monitor a patient’s condition in their own community.

They are specially trained to make sure that if you are diagnosed with heart failure, your symptoms are controlled and you understand your condition and how to manage it. The heart failure nurse will also make sure you are on the best possible medicine for strengthening your heart.

Once the nurse is happy you are stable and on the correct therapy they will transfer your care back to a nurse who is based in the community. The heart failure nurse will see each patient a minimum of 4 times over six months; often more if the symptoms are hard to control.

If at any point your symptoms worsen the heart failure nurse will become involved in your care again.
