Eight steps to the Condition Management Programme
You will meet with an Incapacity Benefit Personal Advisor at the Job Centre.
You will be referred to a Condition Management Practitioner who arranges an appointment to tell you about Condition Management.
You will undergo an assessment. If you both agree that Condition Management is right for you and you wish to continue, then the Practitioner will work with you to design a personalised action plan dependent on your needs.
The GP (and any other health professional involved in care) will be advised that you have volunteered and have given informed consent.
The programme will last approximately 14 weeks.
The Practitioner will see you regularly and the programme can be adapted at any time.
At the end of the programme you will meet with your Practitioner to discuss the benefits of the programme before returning to your Personal Advisor.
In addition to the above, you will also have access to a dedicated 24 hour telephone helpline service.
