What are Community Health Partnerships?

Community Health Partnerships (CHPs) are new organisations being developed across Scotland to manage community health services. CHPs develop existing links between community and hospital-based health provision and local authority services such as social work, housing, leisure and education.

CHPs replace the local healthcare co-operative (LHCC) structure and NHS Lanarkshire Primary Care Operating Division.

The principal aim for CHPs is to improve the long-term health and wellbeing of communities and enhance the quality of health and social care services.

CHPs strengthen staff and public influence in the development and planning of services. Public Partnership Forums (PPFs) ensure that patients, carers, local community groups and voluntary organisations can get involved in the development of services.
