Lanarkshire has two Community Health Partnerships (CHPs): North Lanarkshire and South Lanarkshire.
They were introduced to manage community health services and develop closer partnerships between health, social care and hospital-based services. Their principal aim to improve the long-term health and well-being of communities and enhance the quality of health and social care services. The introduction of CHPs has also strengthened staff and public influence in the development and planning of services. Members of the public can get involved through Lanarkshire's Public Partnership Forums.

North Lanarkshire CHP is split into the following localities: Airdrie, Bellshill, Coatbridge, Cumbernauld, Mothwerwell and Wishaw. It covers the same geographical area as North Lanarkshire Council taking in a population of approximately 326,360.
South Lanarkshire CHP South is split into the following localities: Clydesdale East Kilbride; Hamilton/Blantyre Rutherglen/Cambuslang; and . It covers the same geographical area as South Lanarkshire Council taking in a population of approximately 311,880.
Lanarkshire's two CHPs came into being on 1 April 2006 replacing the local healthcare co-operative (LHCC) structure and NHS Lanarkshire Primary Care Operating Division.
